This group project is intended to improve your understanding of aging as a social process and the core complexities faced by the aging population. This group project will encourage your exploration and understanding of your community and the degree to which it promotes aging-in-place. As well, you will have an opportunity to develop your own plan for an age-friendly community based on your observations and recommendations.

Purpose
This assessment is designed to help you understand the degree to which communities promote the philosophy of ‘aging in place’ while also applying knowledge of the aging process, and what services, supports, and structures advance age-friendly communities. The scope of gerontological social work responsibilities range from direct service provision to community work, to advocacy, all of which can be usefully applied to this exercise as it links micro needs to macro efforts.
Completing this assessment will help you achieve the following learning outcomes:
- Recognize the values of social work (i.e., respect of an individual’s right to dignity and self-determination) and apply it to the various challenges older adults face as they navigate through the aging experience (i.e., death/dying, loss of independence, transition into health-related services).
- Identify the biopsychosocial challenges older adults face and select and apply appropriate methods and interventions based on concepts and theory grounded in research.
- Assess the cognitive, physical, emotional, and psychological changes of older adults and develop appropriate treatment plans as it relates to the individual experiences of the older adult population.
- Define holistic person-centered practice and recognize how associated values/beliefs can enhance the aging experience for older adults and their caregivers.

1. Brief Community Assessment & Findings (Written)
- Identify a GTA or Canadian community of your choice and conduct a scan of that community using the World Health Organization’s Domains of Livability. (Community/health care, transportation, housing, social participation, outdoor spaces, social inclusion, civic participation/employment, communication/information).
- Using these domains as your guide, identify areas where your community is doing well in terms of moving towards age-friendly and areas that could use improvement. Provide a written summary of your findings – in what ways your community meets them, and/or in what ways your community does not meet them. This written section should be 1- 2 1/2 pages max.
